About The Position

Puget Sound Home Health & Hospice is seeking an experienced PRN MSW or LICSW to join our home health team in Kitsap County. Accountability Passion for Learning Love One Another Intelligent Risk Taking Celebration Ownership Home Health Social Worker responsibilities: Assess the psychosocial status of patients related to patient’s illness Develop and provide multiple community and governmental resources to support a patient in their home environment Provides assessment in patient’s residence and carries out social evaluations and plan interventions Participate in the development of the individualized plan of care Assist physician in understanding significant social and emotional factors Participate in quality assessment performance improvement team Serve as liaison between patients and community agencies Maintain and develop contracts with public and private agencies Qualifications: A graduate of an accredited master’s program in social work Licensed MSW or LICSW in Washington State Minimum of one year’s experience in health care Experience with home health or hospice preferred Possesses and maintains current BLS Certification A licensed driver with an insured automobile Visit: http://pugetsoundhh.com/ The employer for this position is stated in the job posting. Requirements

  • A graduate of an accredited master’s program in social work
  • Licensed MSW or LICSW in Washington State
  • Minimum of one year’s experience in health care
  • Possesses and maintains current BLS Certification
  • A licensed driver with an insured automobile
