About The Position

Lutheran Homes & Health Services, an Illuminus Community, has been a part of the local community caring for the families of Fond du Lac and the surrounding area since 1936. Conveniently located in the heart of downtown Fond du Lac, Lutheran Homes & Health Services provides assisted living, skilled nursing and short-term rehab services to older adults. The position is for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in PRN roles with AM, PM, or NOC shifts available. The LPN earns $26.44-33.00 per hour, while the RN earns $40.00 to $45.00 per hour, both dependent on experience. There is a shift differential for PM and NOC shifts. A casual team member (PRN) is required to work six shifts per month; at least three of the shifts must be on a weekend; work one summer and one winter holiday (as available).

Requirements

• Licensed Practical Nurse (LPN) or Licensed Registered Nurse (RN) in the State of Wisconsin.
• Active CPR certification.
• Knowledge and/or experience in geriatrics.
• Knowledge and/or experience with state and federal regulations.

Responsibilities

• Provide prescribed medical treatment under the general supervision of a registered nurse.
• Identify, provide for, and accept responsibility for the fulfillment of resident's total nursing needs.
• Perform nurse practice activities including assessments, medication administration and monitoring, and documentation.
• Ensure the provision of person-centered care.
