It's designed for individuals recovering from serious injuries or illnesses that have affected their ability to function independently. This new, state-of-the-art facility provides intensive, patient-focused rehabilitation services, including physical, occupational, and speech therapy. It specializes in helping patients regain mobility, strength, and independence after events like strokes, traumatic injuries, or surgeries. Requirements

• Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Minimum of 2 years of experience in social work or case management in an inpatient setting preferred.
• Acute rehabilitation hospital experience strongly preferred.
• Effective oral and written communication in English; additional languages preferred.
• Proficiency in Microsoft Office applications required.

Nice To Haves

• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).

Responsibilities

• Coordinate the interdisciplinary care plan.
• Manage discharge planning.
• Communicate patient progress within the inpatient rehabilitation setting.
• Serve as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.
