About The Position

Elara Caring is seeking a PRN Admissions Registered Nurse to join their home care team. This flexible opportunity is ideal for RNs who enjoy admissions, assessments, and helping patients safely transition home after hospitalization. The role involves setting the tone for the patient's journey through patient education, clinical assessments, and building meaningful relationships. Elara Caring delivers high-quality care to over 60,000 patients daily in their homes, with nurses playing a critical role in patient healing and recovery. This PRN position offers the flexibility to choose shifts and provides one-on-one patient care in a home setting, appealing to nurses looking for autonomy and a slower pace than hospital bedside care.

Requirements

  • Current Registered Nurse (RN) license in the state
  • Graduate of an accredited nursing program
  • 1+ year clinical nursing experience
  • Strong patient assessment and documentation skills
  • Ability to travel within the branch coverage area
  • Reliable vehicle, valid driver’s license, and current auto insurance

Nice To Haves

  • Home Health RN or Hospice RN experience
  • Experience with Homecare Homebase (HCHB)

Responsibilities

  • Complete Start of Care and Admission visits for home health patients
  • Perform comprehensive clinical assessments and documentation
  • Initiate the physician ordered plan of care
  • Educate patients and families on medications, treatment plans, and expectations
  • Communicate patient condition and findings with physicians and care teams
  • Identify changes in patient condition and adjust care plans when needed
  • Ensure documentation meets regulatory and quality standards
