About The Position

Elara Caring is seeking a PRN Admissions Registered Nurse to join their home care team. This flexible opportunity is ideal for RNs who enjoy admissions, assessments, and helping patients safely transition home after hospitalization. Requirements

• Current Registered Nurse (RN) license in the state
• Graduate of an accredited nursing program
• 1+ year clinical nursing experience
• Strong patient assessment and documentation skills
• Ability to travel within the branch coverage area
• Reliable vehicle, valid driver’s license, and current auto insurance

Nice To Haves

• Home Health RN or Hospice RN experience
• Experience with Homecare Homebase (HCHB)

Responsibilities

• Complete Start of Care and Admission visits for home health patients
• Perform comprehensive clinical assessments and documentation
• Initiate the physician ordered plan of care
• Educate patients and families on medications, treatment plans, and expectations
• Communicate patient condition and findings with physicians and care teams
• Identify changes in patient condition and adjust care plans when needed
• Ensure documentation meets regulatory and quality standards
