Requirements

Graduate of an accredited Registered Nursing program
Active RN license in the state
2+ years of clinical or home health experience preferred
Strong communication and organizational skills
Commitment to high-quality, patient-centered care
Ability to work collaboratively within an interdisciplinary team
Two weeks of full-time availability required for orientation

Responsibilities

Assess clients to determine eligibility and appropriateness for private duty home care
Complete admissions and develop individualized plans of care
Collaborate with physicians, caregivers, and referral sources to ensure seamless care transitions
Coordinate services and resources to support continuity of care
Partner with schedulers to ensure appropriate caregiver placement
Supervise RNs, LPNs, and Home Health Aides to ensure quality care and accurate documentation
Review clinical documentation to ensure alignment with the plan of care
