About The Position

RVNAhealth is seeking a compassionate Live-In Nurse (LPN preferred; RN considered) to provide one-on-one care in a private home setting. This per diem role supports a patient transitioning from rehab to home. Location: Southbury, CT Start: 2nd week of January (pending patient discharge from rehab) Duration: 1-2 months Schedule: Live-in; 12.5 hours/day (breaks included) Why RVNAhealth? Requirements

  • Graduate of an accredited nursing program
  • Current CT RN or LPN license (LPN preferred) or Compact license required (active and in good standing).
  • Minimum 1 year of nursing experience (hospital or home health preferred)
  • Strong documentation and communication skills
  • Computer literate

Responsibilities

  • Provide direct patient care in the home
  • Complete patient assessments and develop/update plans of care
  • Coordinate care and communicate with physicians
  • Monitor patient status and respond to changes in condition
  • Maintain accurate, timely documentation per RVNA policies
  • Educate patients and families on wellness and safety
  • Follow all agency policies, HIPAA, infection control, and regulatory standards
  • Participate in required in-services and maintain CPR certification
