We serve clients in Cook, Lake, DuPage, Kane, Will, Kendall, and McHenry county. We have a stable client census with consistent assignments and hours, offering you the ability to work one on one with patients and their caregivers long-term. The Private Duty Nurse (PDN)-RN is a member of the client care team that provides services directly, or acts as a placement agency, to deliver skilled nursing services to clients in their personal residences. The PDN RN works under the supervision of the Clinical Manager. Essential Job Responsibilities: Evaluates and treats clients with appropriate level of skill according to client’s needs under the supervision of an Clinical Manager (RN). Assist with providing continuity of care and emotional needs of client and family. Resourceful in meeting client’s psychosocial needs. Verbal demonstration of client rights and responsibilities. Make decisions based on sound nursing judgements. Recognizes and copes with emergencies appropriately. Ensures proper management of client services. Demonstrates competent use of equipment. Appropriately communicates information relative to client care in accordance with agency policies. Observes the client’s general physical, emotional, and mental status, and reports all changes in the client’s conditions, family situation, incident, and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator. Ensure all forms of communication with clients, families, and health plan providers are documented timely to promote continuity of care across all interdepartmental functions. Build relationships with clients to encourage compliance with care plans and client safety. Maintains accurate clinical and shift work documentations to update system in accordance with guidelines set forth by the agency in a comprehensive and timely manner. Manage client/contract satisfaction with the highest level of customer service standards, communication, and documentation. Adheres to infection control policies and exposure control plan as mandated. Monitor and maintain clinical report, including Infection and Occurrence/Complaint Reports. Adheres to and promotes safety and injury prevention measures in client and workplace settings. Maintain and improve skills through attendance with staff development programs and in-service education requirements. Participate in community outreach activities that promote the goals and objectives of the agency. Maintain Confidentiality in all professional areas and consider the needs of clients and their families, other employees, physicians, visitors, and all members of the community in an informative, courteous, and compassionate manner.
