About The Position

We’re hiring Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide personalized, 1:1 care across Suffolk County.

Requirements

• Active LPN or RN license issued by New York State
• Minimum 1 year of experience in a home care setting
• Current CPR/BLS certification
• Strong communication and organizational skills
• Empathy, reliability, and a commitment to high-quality care

Responsibilities

• Deliver direct patient care and treatments based on individualized care plans
• Administer medications and tailored interventions
• Promote wellness through education, counseling, and preventive care
• Collaborate with multidisciplinary teams to coordinate care
• Maintain accurate and timely nursing documentation
• Assist with mobility and physical support (must be able to lift up to 50 lbs)
• Stay active and engaged during extended periods of standing or movement
• Take on additional duties as needed to support patient well-being
