Healthcare is broken — especially in underserved communities like the Bronx, where more than 66% of patients rely on Medicaid. Fragmented systems, outdated workflows, and disconnected data lead to poor health outcomes, clinician burnout, and unnecessary financial strain on both patients and providers. ecares health was born out of this reality. We are a healthcare technology startup spun out of Essen Health Care, one of the largest multispecialty medical groups serving the Bronx and surrounding communities. For more than two decades, Essen has delivered frontline care to tens of thousands of patients across primary care, specialty care, urgent care, home-based services, and population health programs. ecares is built directly from those real-world operations — not theory. We work hand-in-hand with Essen’s physicians, care teams, and healthcare operators to build a modern care delivery platform that connects the entire patient journey; from scheduling and care coordination to lab results, communication, and billing. Instead of rebuilding healthcare from scratch, our venture studio model allows us to integrate and orchestrate best-in-class tools into one unified, patient-centric platform. Our mission is simple but powerful: to bring concierge-level healthcare to underserved communities at scale. Because people in the Bronx deserve the same seamless, high-quality care experience as anyone else. Job Summary: Essen Healthcare is rebuilding ecares, our enterprise care coordination and patient engagement platform, into a next-generation digital health product that will redefine how providers, staff, and patients interact. eCARES sits on top of eClinicalWorks (eCW) and acts as the operational and clinical orchestration layer — bringing together scheduling, care coordination, communication, lab results, and workflow automation into one intuitive, modern platform. Our goal is to eliminate friction, reduce administrative burden, and give both providers and patients a clear, real-time understanding of the patient journey. This is not a maintenance role. This is a platform rebuild, designed to scale beyond Essen to external healthcare organizations. We are seeking a Principal Software Engineer who thrives in startup-style environments, has experience in big tech or venture-backed platforms, and wants to build something that directly impacts patient care.

Requirements

  • 3+ years of software engineering experience
  • Experience in big tech, high-growth startups, or both
  • Strong system design and architecture experience
  • Experience building scalable, production-grade platforms
  • Comfort working across: Frontend Backend APIs Integrations
  • Experience working with product, operations, or non-technical stakeholders
  • The ability to translate real-world business and healthcare problems into technical solutions
  • A strong interest in healthcare, care delivery, and patient experience

Nice To Haves

  • Healthcare experience is a plus — but curiosity and the ability to learn complex systems quickly matters just as much.

Responsibilities

  • Own large-scale features from concept to production
  • Translate operational and clinical requirements into technical solutions
