JOB DESCRIPTION: Provides high quality patient care, displaying good skills in psychiatric counseling. Provides group therapy, individual counseling, family and marriage counseling, and discharge planning. Functions as a member of the multidisciplinary team. Provides diagnostic evaluations and interprets complex psychosocial factors for a variety of psychiatric and addiction patients. Relates accepted theories of psychiatry/psychology to practice situations. Assesses the particular needs of psychiatric and addiction patients and their families when appropriate to the patient's treatment. Assesses conflicts and problems leading to complaints and symptoms that bring individuals to inpatient/outpatient psychiatric/addiction treatment. Provides emergency assessment of potential patients in need of inpatient hospitalization or other appropriate alternatives. Conducts assigned groups and therapy sessions with patients and/or their families and documents per policy. Makes referral to aftercare group therapy for former patients. May conduct therapy sessions for Partial Hospitalization day or evening program or evening Aftercare groups. Ensures that clear treatment plans are concisely written in the patient charts. Writes follow-up progress notes on each patient chart at the completion of each activity or therapeutic exchange as observed, case annotations and entries. Completes all program discharge and aftercare document forms on or before discharge date. Requires instruction only in highly complex, new or specialized methods or procedures. Performs work at an advanced, specialized and independent level. Makes team aware of pertinent information gleaned from interaction with patients and/or their families. Performs other related duties as required.
