As a Primary Support Professional, you will provide essential assistance in the home and community, helping clients navigate daily living tasks while promoting independence and safety. You will collaborate with the Program Coordinator to implement the Individual Support Plan (ISP), ensuring that each client receives tailored care based on their unique needs. Requirements

  • Must be 18 years of age; must have a high school diploma or GED.
  • One year experience in the field of intellectual/developmental disabilities, or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office.
  • Current driver’s license, acceptable driving record, and current auto insurance.
  • Successful completion of a background check, including criminal record, driving record, and abuse/neglect checks.
  • Completion of New Hire Orientation and annual training requirements.
  • ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time).
  • Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
  • Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.

Responsibilities

  • Work with assigned client(s), supporting them in their homes and communities as directed by the Program Coordinator and Regional Director.
  • Participate in ISP meetings and collaborate during RN visits or service monitoring.
  • Train staff in clients’ medical needs and personal preferences to ensure quality support.
  • Oversee daily documentation, including residential notes, health tracking, and checklist completion.
  • Maintain an updated house calendar and ensure all appointments and medical visits are logged.
  • Ensure the home meets health and safety standards, including food storage and cleanliness.
  • Collaborate with the Program Coordinator to develop positive behavioral support strategies as needed.
