About The Position

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards. Position Details: Mercy Clinic Southfork 12700 Southfork Road St. Louis, MO 63128 Schedule: Monday - Friday No weekends or holidays! Must complete three months of on-site training prior to transitioning to hybrid work-from-home schedule; Must also be available to come on-site for monthly clinic meetings and 1-2 times per week Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic. They will have the option to move hybrid after training is complete. LPN Qualifications: Education: Diploma in Practical Nursing Licensure: Active LPN license Experience: 2-3 years of experience preferred RN Qualifications: Education : Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor’s degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor’s degree Licensure: Active RN license Experience: 2-3 years of experience preferred Mercy Key Benefits: Tuition Reimbursement up to $2,000 for continuing education Health/Dental/Vision available after day one Annual contribution of $100 per month to eligible co-workers enrolled in the Dependent Care FSA Paid parental leave for new parents 401k with employer match Paid PTO for volunteering Competitive salary Future career growth! Requirements

• Active LPN or RN license
• Diploma in Practical Nursing (LPN)
• Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor’s degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor’s degree (RN)
• 2-3 years of experience preferred
