Primary Care Provider | Central City Concern Make an Impact Where Care Is Needed Most Provide low- barrier, trauma- informed primary care to individuals experiencing homelessness and complex health challenges. Central City Concern providers practice in flexible, community- based settings that bring care directly to patients. Where You’ll Practice Primary care clinics and shelter- based clinics Mobile health clinics and street medicine settings Community locations serving highly vulnerable populations What You’ll Do Diagnose and treat acute and chronic health conditions Provide integrated care addressing physical health, mental health, and substance use disorders Prescribe medications, including medications for opioid use disorder using a harm reduction approach Deliver wound care and preventative services common in homeless medicine Why This Role Matters Practice innovative, low- barrier medicine for underserved communities Work within a multidisciplinary team supporting whole- person care Help patients build stability, health resilience, and long- term engagement in care Salary Ranges (Full-Time) starting at: Nurse Practitioner $135,000 annually Physician Assistant $135,000 annually
