About The Position

Immanuel Pathways PACE® is seeking a Primary Care Provider NP to work at Pathways Omaha located at 5755 Sorensen Parkway, Omaha, NE 68152. The Nurse Practitioner (NP) provides primary medical care to Immanuel Pathways PACE participants, collaborating in a co-management model with physicians and the interdisciplinary team (IDT) to manage both chronic and acute conditions. The NP completes assessment visits, develops and implements care plans, and ensures adherence to PACE protocols, state, and federal regulations. Additionally, the NP demonstrates high emotional intelligence, professionalism, and a strong commitment to the values and mission of the organization, engaging with participants, their families, the IDT, and PACE leadership in a collaborative, respectful, and effective manner.

Requirements

  • Active Nurse Practitioner license in Nebraska and Iowa.
  • RN license in the state of employment and/or compact state license.
  • Current Board Certification as a Nurse Practitioner.
  • Current, unencumbered DEA registration.
  • Two (2) years treating patients in a hospital, skilled care, or other health-related entity.
  • One (1) year of experience working with the frail or elderly population required, or completion of job-specific training related to working with the elderly population within the first six months of hire.
  • Basic Life Support (BLS) current certification.
  • Valid driver's license, proof of insurance, and reliable means of transportation.

  • Experience in geriatric care.
  • Familiarity with PACE program standards and protocols.

Responsibilities

  • Promote aging in place by preventing unnecessary hospitalizations, ED visits, and nursing home admissions.
  • Provide excellent geriatric care, focusing on participant autonomy and dignity, particularly regarding advanced care planning.
  • Maximize participant well-being through proactive health care maintenance, regular visits, and care plan adjustments based on life expectancy and frailty.
  • Support caregivers, reduce caregiver burden, and ensure early family involvement in care planning.
  • Document all Hierarchical Condition Codes (HCCs) and identify previously missed diagnoses to enhance care outcomes.
  • Provide primary care to participants within an MD/NP co-management model, ensuring alignment with best practices, clinical protocols, and relevant guidelines.
  • Complete initial, semi-annual, transition of care, 60-day assessments, and acute or unscheduled visits as clinically indicated.
  • Develop and implement patient-centered care plans, focusing on both acute and chronic conditions, in collaboration with the IDT.
  • Manage chronic conditions and acute illnesses, with an emphasis on holistic, evidence-based care that aligns with participant goals and preferences.
  • Engage in reoccurring goals of care conversations, especially during acute changes, and incorporate motivational interviewing techniques.
  • Provide preventive care (immunizations, screenings, health maintenance) and health education, empowering participants and caregivers.
  • Prescribe medications and make referrals to specialists judiciously, ensuring personalized care.
  • Manage care in contracted settings (nursing homes, hospitals, assisted living), ensuring high-quality visits, communication, and care coordination.
  • Conduct honest conversations with participants and families about care priorities, advanced directives, and end-of-life considerations.
  • Act as a resource during the intake of new participants, helping to ensure their care protocols and needs are understood and addressed from the start.
  • Exhibit professionalism and emotional intelligence in daily interactions with the interdisciplinary team (IDT) and PACE leadership.
  • Support a culture of continuous improvement by being receptive to feedback, fostering professional growth, and maintaining a calm, respectful presence in challenging situations.
  • Ensure each participant is seen at least every 60 days and see each participant within 7 days of an ED visit.
  • Make efforts to see patients face-to-face before sending them to the emergency room unless it's an acute emergency.
  • Willingness to see patients in their homes as needed and via telepresence as needed.
