Transportation is provided to patients that need it at no charge.

Requirements

• Board Certified in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
• Clear and active Georgia medical license.
• 2+ years of outpatient primary care experience (preferred).

Responsibilities

• Provide primary care to a patient panel of seniors with 3-5 chronic diseases.
• Manage a maximum of 450 patients.
• Work Monday to Friday from 7:30 AM to 5:00 PM, with the last patient scheduled at 4:15 PM.
• Participate in value-based care initiatives.
• Collaborate with a dedicated care and support team, including a Medical Assistant and Scheduler.
• Ensure work/life balance with administrative time included in the schedule.
