Join Senior Doc Medical Group, where we’re redefining how seniors experience healthcare. We’re seeking a Board-Certified or Board-Eligible Physician in Internal Medicine, Family Medicine, or Geriatrics to deliver comprehensive primary care to aging adults across home-based and community settings in San Jose, CA.

  • Serve as the primary care provider for assigned patients, managing preventive, acute, and chronic health needs.
  • Perform comprehensive health assessments, order and interpret diagnostics, and guide personalized care plans.
  • Collaborate with an interdisciplinary team of Nurse Practitioners, Physician Assistants, and Wellness Coordinators.
  • Lead population health initiatives and quality improvement programs that enhance patient outcomes.
  • Provide mentorship to mid-level providers and clinical support staff.
  • Participate in regular provider meetings and professional development to advance clinical excellence.
  • MD or DO with an active California medical license.
  • Board Certified in Family Medicine, Internal Medicine, or Geriatrics.
  • Skilled in EMR documentation and team-based collaboration.
  • Compassionate, communicative, and driven by patient-centered outcomes.
  • Experienced in senior or geriatric care (preferred).
  • Monday–Friday schedule – no nights or weekends required.
