Provide comprehensive primary care services, including preventative care, acute and chronic disease management, substance use, hypertension, and diabetic care. ? Conduct thorough physical examinations and age-appropriate screenings according to clinical guidelines. ? Diagnose, treat, and manage conditions such as hypertension, diabetes, asthma, COPD, mental health disorders, musculoskeletal issues, infections, and minor injuries. ? Develop and implement personalized treatment plans, involving patients in decision-making and goal setting. ? Order and interpret laboratory tests, imaging studies, and other diagnostics to inform treatment. ? Prescribe medications responsibly, monitor efficacy and side effects, and adjust treatments accordingly. ? Provide lifestyle counseling (e.g., smoking cessation, nutrition, exercise) to prevent disease and promote wellness. ? Identify patients who require specialist care and facilitate appropriate referrals and care coordination. ? Follow up on patient progress, review test results, and ensure proper follow-up care. ? Proficient in Women's health procedures helpful. ? Perform in office primary care procedures, including joint injections and minor dermatologic procedures helpful. ? Provide competent chronic disease management and geriatric medical management. ? Respond to urgent patient issues during office hours. ? Document all patient interactions in a timely, thorough, and compliant manner using Electronic Medical Records (EMR) systems. ? Participate in quality improvement initiatives, audits, and peer reviews to ensure adherence to evidence-based standards. ? Collaborate with a multidisciplinary team of nurses, case managers, behavioral health professionals, and social workers. ? Comply with all HIPAA, OSHA, and regulatory guidelines governing patient care and safety. ? Engage in continuing medical education (CME) and stay current with advancements in medicine, clinical protocols, and health policy changes. ? Reporting of Adverse Incidents.

Requirements

  • Medical Degree (MD or DO) from an accredited medical school.
  • Completion of an accredited residency in Family Medicine or Internal Medicine.
  • Board Certification or active pursuit of certification in primary care specialty.
  • Unrestricted and valid WI state medical license.
  • Valid DEA license for prescribing medications.
  • Current BLS/ACLS certification or past certification with renewal requirement upon hire.
  • Medicaid and Medicare provider eligible required.
  • Ability to maintain confidentiality and observe all requirements of the Federal Privacy Act and HIPAA regulations.
  • Valid Drivers License.
  • Valid Vehicle Insurance .
  • Be able to pass pre-employment and random drug testing.
