This is a 100% outpatient, value-based primary care role serving seniors with multiple chronic conditions in a mission-driven, team-supported environment designed for sustainability, quality, and physician autonomy. Compensation & Partnership • Base Salary: $265,000 • Signing Bonus: Up to $50,000 • Annual Bonus: Up to $20,000 in Year 1 (increases upon partnership) Partnership Track: • No buy-in required • Typically achieved within 12–18 months • 65% of eligible physicians achieve partnership after Year 1 • Partner earnings commonly exceed $300,000 annually Practice Model • 100% Outpatient • 18 patients per day • Small patient panel (maximum 450 patients) • Senior population with 3–5 chronic conditions • Value-Based Care environment • Administrative time built into schedule • Monday–Friday schedule • Clinic hours: 7:30 AM – 5:00 PM • Last patient scheduled at 4:15 PM • No nights • No weekends • Strong work/life balance Designed to reduce physician burnout while improving patient outcomes.
