Physician Affiliate Group of New York (PAGNY) is adding a Primary Care Physician to the group at NYC Health + Hospitals/Gotham Health, Cumberland. Opportunity Details: Performs medical services within the scope of approved privileges. Document all patient contact in the EMR in a timely manner. Assumes responsibility for patient care. Collaborates to provide care coordination activities in providing comprehensive care (e.g., other healthcare providers, local health departments, social service, nutritionist, dentist, community-based organizations, home care and patient education groups). Provide relevant health education and instructions, counsels patients/family responsible on the impact of illness and principles of health promotion and maintenance. Provides clinical and didactic instruction to professional staff, peers, students and community groups and conducts in-service educational programs, as needed. Participates in departmental meetings and interdisciplinary community committees, as well as peer review performance improvement meeting activities. Follows Gotham, NYC Health + Hospitals and facility-specific policies and protocols. Maintains expected standards of patient information confidentiality as per HIPAA policy. Ensures credentials are maintained. Maintain patient safety and infection control protocol as per policies. Required to complete annual evaluations. Participate in committees by your Medical Director. Available to work evenings and weekends. May be asked to move between sites as designated by Chief Medical Officer. Peer review. Refer to specialties, e.g. Dental. Care gaps/Care gap metrics. Completion of assigned work ques in EPIC. Completion of patient forms. Perform administrative, professional and consulting duties as requested by the Medical Director.

  • Annual Base Salary: $236,000 (board eligible); $241,000 (board certified) based on 40-hour work week.
  • Additional compensation available for extra call or sessional/per diem hours.
  • The annual total value of the compensation package is estimated at $315,120 (board eligible) and $320,620 (board certified) , which includes the baseline salary, 401 (k) contribution, sessional/per diem hours, and other factors as set forth below:
  • Estimated annual compensation for 10 additional sessional/per diem hours worked per week is $55,520.
  • 401(k) Company Contribution (subject to IRS contribution limits): Employees are immediately vested in a 3% company contribution of base earnings. No employee match is required. After one year of service, employees receive an additional 7% company contribution of base earnings. No employee match is required.
