About The Position

Outpatient primary care role serving patients ages 16 and older
Locations in Columbus, Cleveland, and Cincinnati
Value based care model focused on quality and continuity
Manage a balanced daily schedule averaging approximately 8 to 12 patients
Mix of scheduled visits and same day or urgent care coverage
Team based environment with integrated behavioral health and care coordination
Modern outpatient clinical setting
Monday through Friday daytime schedule
No nights, no weekends, no call

Requirements

• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Ohio medical license or eligibility for licensure

Nice To Haves

• Experience working with complex or underserved populations preferred
• Experience with behavioral health or addiction care a plus
