A Day in the Life of this Role: Reporting to the Site Medical Director, the Primary Care Physician serves as the clinical leader of an interdisciplinary care team. The Physician provides comprehensive and continuing care to patients requiring routine and emergency treatment at health centers or other locations within OCHS’s scope of practice, including skilled nursing facilities. Additionally, the incumbent collaborates deeply with the care team to coordinate referrals, support health promotion, and manage chronic conditions effectively.

Requirements

  • Requires an M.D. or D.O degree, board certification or eligibility in an appropriate specialty.
  • Possess and maintain a valid and unlimited license to practice medicine in the Commonwealth of Massachusetts.
  • Cardiopulmonary Resuscitation (CPR) certification.
  • Possess a valid federal Drug Enforcement Administration number and any other required state public health certifications with respect to prescribing privileges.
  • Certified provider and accepts assignment under the Medicare and Medicaid programs, and a qualified provider with respect to such other third-party payers with whom the Health Center may do business.
  • Is not excluded by state or federal authorities from participating in any health care programs, including Medicare and Medicaid.
  • Is not penalized with civil monetary fines or assessments, or administrative or criminal sanctions for a material violation of any federal or state law involving Medicare, Medicaid, or any other federal or private third-party reimbursement program.
  • Maintains full privileges as a member in good standing of the Health Center’s Medical/professional staff and comply with all by-laws and rules and regulations thereof.
  • Demonstrated commitment to the principles of Team-Based Care and the Patient-Centered Medical Home (PCMH) model.
  • Strong leadership skills with the ability to mentor, delegate, and trust team members to perform their roles.
  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
  • Excellent written and verbal communication skills – bi-lingual helpful.
  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
  • Excellent judgment and ability to solve problems in a timely manner.
  • Ability to adhere to strict confidentiality standards.
  • Demonstrated ability to work effectively both as a team leader and a team member.
  • Demonstrated ability to work in a culturally diverse and inclusive environment.

Responsibilities

  • Lead and collaborate within an assigned Care Team (including RNs, LPNs, Medical Assistants, Behavioral Health Clinicians, and Patient Access staff) to manage the health of a patient panel.
  • Champion a culture of shared responsibility for patient outcomes.
  • Participate in and/or lead daily team "huddles" to review the schedule, anticipate patient needs, identify care gaps, and coordinate workflow before the clinic day begins.
  • Provide clinical oversight and mentorship to other health care professionals such as registered nurses, medical assistants, phlebotomists, and allied health staff, ensuring they are empowered to perform clinical tasks at the top of their licensure and training.
  • Actively collaborate with Integrated Behavioral Health providers and Community Health Workers to address social determinants of health and mental health needs within the primary care setting.
  • Collaborate with the Site Director and nursing staff regarding daily flow.
  • Demonstrate willingness to accept emergency, urgent, and other walk-in patients and utilize team resources to manage such requests efficiently.
  • Keep accurate, complete, legible, and timely medical records that serve as a communication tool for the broader care team, as required by OCHS’s policy and law.
  • Participate in patient care audits for quality assurance and utilization review to maintain and improve patient care standards of excellence.
  • May provide administrative management input for staff performance reviews, hiring decisions, and disciplinary issues with a focus on team dynamics and collaboration.
  • Participate in the on-call schedule as assigned including coverage of weekend shifts as needed.
  • May participate in coverage for skilled nursing facilities within the scope of practice.
  • Maintain knowledge of current developments in specialty to enhance professional expertise including maintenance of Board Certification if applicable and completing CME as needed to maintain licensure.
  • Maintain a highly collaborative and professional working relationship with other physicians and health care professionals within the Health Center and OCHS administrative and support staff.
  • Participate in regular provider and staff meetings as set by the Chief Medical Officer.
  • Perform other related duties as reasonably required or assigned.
  • Ensure that participation in activities outside of the Health Center does not interfere with duties hereunder.
  • Other related duties and projects as assigned.
