About The Position

Humana's Primary Care Organization, including Conviva/CenterWell Primary Care, is one of the largest and fastest-growing senior-focused, value-based care providers in the country. This full-time role is for a compassionate, experienced clinician committed to improving outcomes for adult and geriatric populations in a collaborative, patient-centered environment. The position reports to the area medical director. The team-based care model focuses on whole-person care (physical, emotional, social) with a lower daily patient volume to foster deeper relationships and personalized care. Conviva Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment, utilizing an innovative, value-based approach to ensure optimal care at the lowest cost. Requirements

  • MD or DO from an accredited medical school
  • Active, unrestricted medical license in the state of practice
  • Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Two years' experience in care or managing high-acuity geriatric patients
  • Commitment to improving patient experience and outcomes
  • Participation in Tuberculosis (TB) screening program
  • Passionate about contributing to an organization focused on improving consumer experiences
  • Willing to participate in limited on-call coverage by phone

Nice To Haves

  • Specialty training in family medicine, internal medicine, med-peds, or geriatrics
  • Experience working with senior populations or in value-based care settings
  • Proficiency with electronic health records (EHR) and digital documentation
  • Adapt to evolving clinical environments
  • Experience supervising advanced practice providers

Responsibilities

  • Deliver comprehensive outpatient care to adult and senior patients
  • Maintain accurate medical records and documentation
  • Diagnose and manage moderately to complex medical conditions
  • Coordinate referrals and collaborate with specialist
  • Participate in clinical quality improvement projects
  • Work with interdisciplinary teams to ensure holistic care
  • Contribute to strategic programs and innovations in care delivery
  • Participate in limited on-call coverage by phone
  • Contribute to a culture of innovation and continuous improvement
