We’re seeking a Board-Certified or Board-Eligible Physician in Internal Medicine, Family Medicine, or Geriatrics to deliver comprehensive primary care to aging adults across home-based and community settings in San Diego, CA.
Requirements
- MD or DO with an active California medical license.
- Board Certified in Family Medicine, Internal Medicine, or Geriatrics.
- Skilled in EMR documentation and team-based collaboration.
- Compassionate, communicative, and driven by patient-centered outcomes.
Nice To Haves
- Experienced in senior or geriatric care (preferred).
Responsibilities
- Serve as the primary care provider for assigned patients, managing preventive, acute, and chronic health needs.
- Perform comprehensive health assessments, order and interpret diagnostics, and guide personalized care plans.
- Collaborate with an interdisciplinary team of Nurse Practitioners, Physician Assistants, and Wellness Coordinators.
- Lead population health initiatives and quality improvement programs that enhance patient outcomes.
- Provide mentorship to mid-level providers and clinical support staff.
- Participate in regular provider meetings and professional development to advance clinical excellence.
