About The Position
Primary Care Physician $315K Value-Based Care Tampa Bay Multi-Site 450 Patient Panel Not a volume-driven PCP role. Panel capped at ~450 patients with full accountability for outcomes. Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring Primary Care Physicians across the Tampa Bay market in a value-based care model focused on senior populations. AVAILABLE LOCATIONS • Clearwater • Kenneth City • Largo • Lealman • South St. Petersburg • West Largo CARE MODEL • Panel size approximately 450 patients • 18–21 patient visits per day • Value-based care (NOT RVU-driven) • Outcomes measured: admissions, patient experience, and quality metrics
Requirements
- MD or DO required
- Board Certified or Board Eligible in Internal Medicine or Family Medicine REQUIRED
- Active, unrestricted medical license REQUIRED
- Active DEA license REQUIRED
- Minimum 2+ years of Primary Care Outpatient experience REQUIRED
Responsibilities
- Manage acute and chronic conditions in senior population
- Maintain full accountability for patient outcomes
- Lead and coordinate a multidisciplinary care team
- Collaborate with specialists and hospitalists for continuity of care
- Drive patient engagement and behavior change
- Participate in quality and risk management programs
