Transportation is provided to patients that need it at no charge.

Requirements

• Clear/Active medical license.
• Board Certified/Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active DEA.

Nice To Haves

• Experience in managing a small patient panel.
• Interest in holistic health services such as acupuncture, Tai Chi, and nutrition.

Responsibilities

• Provide primary care to a panel of seniors with 3-5 chronic diseases.
• Manage a maximum of 450 patients.
• Work Monday to Friday, with clinic hours from 7:30 AM to 5:00 PM.
• Schedule administrative time within the work schedule.
• Ensure a work/life balance with no nights or weekends.
• Participate in value-based care initiatives.
