Primary Care Plus (PCPlus), a value-based care delivery organization, is seeking Board-Certified/Board-Eligible Internal Medicine or Family Medicine Physicians to join our growing team across Louisiana. Our mission is to provide a path of well-being for our patients through high-quality, cost-effective, and coordinated care that delivers superior outcomes and satisfaction.

Requirements

• M.D. or D.O.
• Active, unrestricted Louisiana medical license.
• Valid DEA and CDS licenses (or ability to obtain).
• Board Certified/Eligible in Family or Internal Medicine.

Nice To Haves

• Basic Cardiac Life Support (BCLS) certification.
• 1+ years of clinical experience in adult, family, or geriatric care.

Responsibilities

• Provide high-quality, cost-effective, and coordinated care to patients.
• Work in a physician-led, team-based environment with support from APRNs, RNs, and a dedicated Quality Improvement team.
• Focus on quality care metrics and patient outcomes.
• See 10-16 patients per day with 30-60 minute appointments.
• Participate in robust transitional care and high-risk outreach programs.
• Receive consistent feedback on quality measures, diagnostic accuracy, and hospitalization rates.
