About The Position

ChenMed, a physician-led and mission-driven primary care organization, is currently one of the most successful full-risk Medicare Advantage providers in the nation. Our mission is to honor seniors with affordable VIP care that delivers better health. We are looking for a Primary Care Physician (PCP) who demonstrates accountability for outcomes, acts as a health coach, and simplifies actions for patients. The PCP will manage a panel of up to 450 patients, focusing on building trusting relationships and helping patients achieve their health goals. The role requires collaboration with care teams and adherence to departmental goals and regulatory compliance.

Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
• Must be able to obtain a State Medical License or already have a current, active State Medical License.
• Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred.
• Current DEA number for schedule II-V controlled substances.
• Basic Life Support (BLS) certification required within the first 90 days of employment.

Nice To Haves

• Board Eligibility is required.
• Fluency in other languages may be required in some centers.

Responsibilities

• Independently provide care for patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes.
• Engage with hospitalists for hospitalized patients.
• Lead a care team consisting of a medical assistant, care facilitator, and care coordinator.
• Participate in risk and quality management programs.
• Recruit patients for the center and additional providers for the company.
• Perform other duties as assigned.
