About The Position

ChenMed, a physician-led and mission-driven primary care organization, is currently one of the most successful full-risk Medicare Advantage providers in the nation. Our mission is to honor seniors with affordable VIP care that delivers better health. We are looking for a Primary Care Physician, Partner (PCPP) who demonstrates accountability for outcomes, acts as a health coach, and simplifies actions for patients. The PCPP will work closely with care teams and is expected to champion a positive culture of love, accountability, and passion within the medical centers.

Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
• Current, active MD licensure in State of employment is required.
• A minimum of 6 years clinical experience in geriatric, adult or family practice setting, including 2 years of ChenMed PCP, Senior Partner experience required.
• Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
• Must have a current DEA number for schedule II-V controlled substances.
• Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employment.

Responsibilities

• Independently provide care for patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes.
• Engage with hospitalists for patient care in hospitals.
• Lead a care team consisting of a medical assistant, care facilitator, and care coordinator.
• Participate in risk and quality management programs.
• Supervise and collaborate with Advanced Practice Practitioners (APPs).
• Recruit patients for the center and additional providers for the market.
