Requirements

MD or DO (Board Eligible/Certified in Internal Medicine, Family Medicine, or Geriatrics)
Active (or eligible) Virginia medical license & DEA
Passion for value-based, patient-centered care

Responsibilities

Provide comprehensive primary care to older adult patients
Take ownership of outcomes while delivering personalized, relationship-driven care
Serve as a health coach, guiding patients toward sustainable lifestyle changes
Coordinate specialty and hospital care while maintaining continuity
Lead and collaborate with your care team
Supervise and support Advanced Practice Providers (APPs)
Partner with leadership on quality, experience, and cost goals
