Requirements
- Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine and/or Geriatric Medicine
- Valid, unrestricted FL license (or willing to obtain)
