About The Position

Gales Ferry Medical Group, a trusted independent medical practice serving southeastern Connecticut since 1970, is seeking a Nurse Practitioner (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner) to join our team. The role involves providing comprehensive primary care services, including sick and well visits and some gynecologic care, to a diverse patient population.

Requirements

• Certification: Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
• Active Nurse Practitioner license in Connecticut (or eligibility to obtain one)
• Minimum of 1-2 years of primary care or outpatient experience preferred but open to stellar new graduates
• Strong communication skills and a commitment to patient-centered care.

Responsibilities

• Conduct comprehensive patient assessments, including routine sick and well visits.
• Provide gynecologic care as part of primary care services.
• Diagnose and manage acute and chronic conditions with evidence-based treatment plans.
• Promote preventive care and health maintenance strategies tailored to patient needs.
• Collaborate with physicians and other healthcare professionals to ensure high-quality, coordinated care.
• Maintain accurate and timely documentation in compliance with healthcare standards.
