About The Position

Elderberry Health is a patient-centered primary care organization serving rural communities in Middle and East Tennessee. We are committed to expanding access to quality primary care through innovative care delivery models, including telehealth, walk-in visits, and home-based primary care. Our small but dedicated team combines the personal touch of a rural practice with modern healthcare technology to deliver exceptional patient outcomes. We are seeking an experienced Physician Assistant (PA-C) or Nurse Practitioner (NP) to join our team as a 1099 independent contractor. This is a remote-first position with a flexible schedule that includes evening and weekend availability. You will provide primary care services across multiple care settings, working collaboratively with our medical assistants and supervising physician.

Requirements

  • Active, unrestricted Tennessee license as a Physician Assistant (PA-C) or Nurse Practitioner (APRN)
  • Valid DEA registration (or ability to obtain prior to start)
  • Minimum of 2 years of clinical experience in primary care, family medicine, or internal medicine
  • Experience with telehealth/virtual care delivery
  • Comfortable working independently in a remote environment with physician oversight
  • Strong clinical judgment and excellent communication skills
  • Reliable high-speed internet connection and dedicated workspace for telehealth visits

Nice To Haves

  • Experience with home-based primary care or house call medicine
  • Familiarity with eClinicalWorks (eCW) EHR
  • Experience with value-based care models or quality metrics (HEDIS, MIPS)
  • Current BLS/ACLS certification

Responsibilities

  • Conduct telehealth visits for acute, chronic, and preventive care
  • Provide walk-in clinic coverage for same-day patient needs
  • Collaborate with Medical Assistants delivering home-based primary care to patients in our service area
  • Perform comprehensive assessments, develop treatment plans, prescribe medications, and order diagnostic tests within scope of practice
  • Document all patient encounters accurately and in a timely manner in the electronic health record (eClinicalWorks)
  • Coordinate referrals and follow-up care with specialists and community resources
  • Participate in quality improvement initiatives and care gap closure activities
  • Maintain compliance with all applicable state and federal regulations, including Tennessee prescriptive authority requirements
