About The Position

Fairview Health Services is seeking a Nurse Practitioner to join their primary care practice, splitting time between two clinics: M Health Fairview Clinic - Brooklyn Park and M Health Fairview Clinic - Bass Lake. The role involves a full-time equivalent (FTE) of 1.0, with 0.75 FTE at Brooklyn Park and 0.25 FTE at Bass Lake. The clinics emphasize a multidisciplinary team approach to managing clinical care, utilizing a fully integrated Electronic Medical Record system (EPIC).

Requirements

• Graduate of an accredited Physician Assistant program.
• Board Certified/Board Eligible through NCCPA.
• Licensed or ability to obtain a PA license with prescriptive authority through the Minnesota Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges.
• 2 years of Physician Assistant experience required.

Responsibilities

• Provide primary care services to patients at both Brooklyn Park and Bass Lake clinics.
• Manage patient encounters, averaging 18-22 patients per day.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Utilize the Electronic Medical Record (EPIC) for patient documentation and management.
