Acts is currently seeking qualified candidates for our Primary Care Office Nurse (LPN) position. In this role, you will be responsible for supporting the overall health and well-being of residents by delivering high-quality, person-directed care and nursing services within the Primary Care Office. This role is essential in advancing the Acts Signature Well-Being Model by contributing to interdisciplinary care, supporting proactive health monitoring, and facilitating access to well-being resources. Collaborates closely with the Nurse Practitioner and the broader wellness team to foster a holistic, resident-centered approach that promotes independence, dignity, and thriving in place.

Requirements

  • Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing
  • Minimum of two years of clinical nursing experience, preferably in a senior living, primary care, or wellness-focused environment
  • Current certification in CPR, AED, and first aid (or obtain within three months of employment)

Nice To Haves

  • Prior experience in office management, care coordination, or leadership roles is strongly preferred
