PRIMARY CARE NURSE - LPN - Blue Team Do you want to utilize assessment and teaching skills to foster patient health? Are you looking for benefits effective on the first day of employment? In this role you will: Triage patients by telephone or in person to determine urgency and level of care needed, assuring appropriate care disposition. Perform patient procedures, assesses patient health status, and provides health education to patients/families regarding pertinent health issues, clinical conditions, treatments, recommended follow up actions and /or home care. Communicate pertinent abnormal test results to provider. Track patients for necessarily follow up documenting appropriately. Utilize those skills to interact with patients/clients, team members and staff from other departments.

Requirements

  • Graduate of accredited nursing program
  • Current Mass. Licensure
  • BLS certification
  • A desire to work as a collaborative member of a multi-disciplinary team.

Nice To Haves

  • Bi-lingual in Spanish /Khmer/Russian with knowledge of medical terminology preferred.
  • Previous Community Health or Ambulatory Experience preferred.
