About The Position
Life Medical is a mobile care practice committed to delivering high-quality, on-demand medical services directly to patients in Assisted Living Facilities. We specialize in Primary Care visits for individuals who require timely medical attention but face barriers to accessing traditional clinics or emergency departments. Our team is passionate about improving access, reducing unnecessary hospitalizations, and providing compassionate care where it’s needed most. We are seeking a skilled and compassionate Primary Care Nurse Practitioner (NP) to join our Mobile Care team. This full-time role involves traveling to Assisted Living Facilities in and around Somerset, NJ and surrounding areas to manage chronic and acute conditions and build meaningful relationships with patients and their families. You’ll work independently with flexible scheduling, supported by a collaborative clinical team.
Requirements
- Licensed Nurse Practitioner in the state of New Jersey
- Board-certified in Family Medicine or Adult-Gerontology
- Minimum of 1 year of clinical experience (geriatrics preferred)
- Strong diagnostic and clinical decision-making skills
- Excellent communication and interpersonal abilities
- Valid driver’s license and reliable transportation
Responsibilities
- Conduct primary care visits in Assisted Living Facilities
- Perform physical exams, diagnostic assessments, and routine screenings
- Manage chronic conditions and acute illnesses
- Prescribe medications and develop individualized care plans
- Coordinate with other healthcare providers and community resources
- Document visits and maintain accurate medical records
- Educate patients and caregivers on health management and preventive care
- Communicate with facility staff, patients, and families regarding car
