Requirements

  • Active and unrestricted Arizona license as a Nurse Practitioner or Physician Assistant
  • Valid DEA license without restrictions
  • Eligible for credentialing with managed care and commercial insurance plans
  • Completion of US-Based NP/PA Program (Not Puerto Rico)

Responsibilities

  • Provide comprehensive outpatient primary care to patients across all age groups
  • Conduct physical exams, obtain medical histories, and perform diagnostic evaluations
  • Manage chronic conditions such as diabetes, hypertension, asthma, and more
  • Design individualized treatment plans aligned with patient goals and clinical guidelines
  • Educate patients and families on disease prevention, management, and wellness
  • Collaborate with physicians, care coordinators, and clinical support staff
  • Document patient encounters accurately and promptly in the electronic health record (EHR)
  • Maintain current knowledge of clinical guidelines and best practices
  • Participate in continuing education and meet all licensure and certification requirements
