About The Position

Alice Hyde Medical Center (AHMC) is seeking a passionate Primary Care Nurse Practitioner or Physician Assistant to join our Dwyer Health Center in Moira, NY. This is a full‑time outpatient opportunity in a collaborative, community‑focused primary care practice. Prior experience is preferred, but new graduates are welcome. What You’ll Do Deliver comprehensive outpatient primary care, including preventive visits and chronic disease management. Assess, diagnose, and treat patients across the lifespan. Order and interpret labs and diagnostic tests. Prescribe medications and develop evidence‑based care plans. Coordinate referrals and collaborate with physicians and the interdisciplinary care team. Support care coordination and continuity to improve patient outcomes. Requirements

  • Active licensure (or eligibility) as a Nurse Practitioner or Physician Assistant in New York State.
  • Strong clinical and communication skills.
  • Ability to work both independently and within a team‑based care model.
  • Commitment to patient‑centered, compassionate care.
  • Nurse Practitioners (NP): Graduate of an accredited master’s, post‑master’s, or doctoral NP program (CCNE or ACEN).
  • Current national certification (e.g., ANCC or equivalent).
  • Physician Assistants (PA): Graduate of an ARC‑PA–accredited PA program (or qualifying predecessor).
  • Current NCCPA certification.

Nice To Haves

  • Primary care experience preferred; new graduates will be considered.
