About The Position

Join our client’s team in a fulfilling role dedicated to helping seniors live independent and fulfilling lives. In this position, you'll be a key part of a caring team, working to create personalized care plans that help improve health outcomes for the elderly in their PACE program. You’ll treat patients at various locations, including in-home and virtual settings.

Requirements

• Minimum 2 years of experience as an RN with complex populations before becoming an NP.
• Must have an active NP license and CPR certification in California.
• Master's degree from an accredited NP program is required.
• Must be bilingual in English and Spanish.
• Should have transportation available for work purposes with a valid CA driver's license.

Responsibilities

• Provide direct medical care to PACE program participants.
• Conduct assessments, manage care plans, and monitor participant health.
• Prescribe medications and other treatments with proper approval.
• Document interactions and care steps in electronic medical records promptly.
• Coordinate referrals to specialists and manage follow-ups.
• Conduct physical examinations, educate participants, and manage chronic conditions.
• Work collaboratively with a multidisciplinary team.
