Requirements
- Current New Hampshire Nursing License (RN or LPN) required.
- Current BLS certification required.
- Strong organizational, communication, and clinical assessment skills.
- Compassionate, adaptable, and team-oriented mindset.
Nice To Haves
- Experience in primary care or ambulatory health preferred.
Responsibilities
- Rotate among all primary care teams to provide hands-on and telephonic patient care.
- Collaborate with providers, MAs, and multidisciplinary staff to coordinate care and ensure smooth transitions.
- Perform clinical duties such as patient education, care coordination, medication review, and follow-up.
- Manage test results, referrals, and care plans in collaboration with the clinical team.
- Deliver compassionate, patient-centered care — every interaction, every day.
