As a Licensed Practical Nurse (LPN) in our primary care practice setting (HomeMD), you’ll report to the Primary Care Practice Manager, and play a vital role in ensuring smooth daily operations while serving as a trusted point of contact for patients, families, and clinical staff. This is an office-based nursing role ideal for an LPN who thrives in a fast-paced environment, values teamwork, and takes pride in delivering excellent service behind the scenes. How You’ll Contribute The LPN supports clinical and administrative operations to ensure patients receive timely, coordinated care. In this role, you will:

Requirements

  • Graduate of an accredited Licensed Practical Nurse (LPN) program
  • Excellent verbal, written, and organizational skills
  • Ability to thrive in a fast-paced, team-oriented environment
  • Proven customer service skills within a healthcare setting
  • Strong interpersonal skills with the ability to communicate clearly and compassionately with physicians, patients, and families
  • Ability to manage competing priorities, work under pressure, and meet deadlines
  • Proficiency in MS Outlook and healthcare-related systems and databases
  • Sound independent judgment and the ability to manage highly confidential information

Nice To Haves

  • A minimum of one year experience in a Primary Care setting, preferred

Responsibilities

  • Triage incoming phone calls with professionalism, compassion, and clinical judgment
  • Order prescriptions and support medication coordination
  • Serve as a key liaison between physicians, patients, families, and administrative staff
  • Assist with administrative and operational tasks to maintain efficient practice workflows
  • Handle confidential medical information with discretion and sound judgment
  • Support a collaborative, patient-focused environment where teamwork is essential
