About The Position

Fairview Health Services is seeking an Internal Medicine physician to join our primary care practice at M Health Fairview Clinic - Brooklyn Park. This is a busy practice with over 12 specialists and many onsite support services. As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care. The practice consists of a 13 provider primary care team with more than 12 specialists onsite. Patient encounters average 18-22 patients per day, with 34 patient contact hours per week. The call frequency is approximately 1 per month, which is phone consultation only with first tier nurse triage. We are looking for a 1.0 FTE (Full Time) or less with a flexible schedule.

Requirements

• Graduate of an accredited ACGME/AOA in Internal Medicine/Pediatrics or residency training program.
• BC/BE with the ABMS or AOA.
• Licensed or ability to obtain Minnesota Board of Medical Practice Licensure.
• Ability to obtain and/or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges.

Responsibilities

• Manage clinical care using a multidisciplinary team approach.
• Conduct patient encounters averaging 18-22 patients per day.
• Maintain 34 patient contact hours per week.
• Participate in phone consultations for call frequency of approximately 1 per month.
