About The Position

AHN Primary Care Institute is seeking a Geriatric Medicine Physician to join our growing team in Erie, Pennsylvania. In this role, you will deliver high-quality, person-centered care in a consultative/co-management capacity to older adults with complex medical and social needs. Highlights: Work closely with primary care clinicians and specialty providers, where you will help guide care plans that promote independence, dignity, and quality of life for aging patients. Practice in a supportive, collaborative environment with access to multidisciplinary team-based resources to ensure coordinated, comprehensive care for every patient. Primary outpatient, with potential for limited practice in skilled nursing facilities/assisted living communities.

Requirements

• MD or DO degree from an accredited medical school.
• Board Certification in Geriatric Medicine.
• Current Pennsylvania medical licensure or eligibility to obtain licensure.
• Experience in geriatric care is preferred.
