About The Position

The Prevention Case Manager plays a vital role in supporting people in the community who are at imminent risk of homelessness. This position involves providing direct, time-limited services to participants, including comprehensive assessments of their needs and connecting them with necessary resources such as legal assistance, housing options, and mental health support. The role emphasizes advocacy to ensure participants receive the full spectrum of services required to achieve housing stability. In addition, the Prevention strategies to prevent participants from re-entering crises or the justice system unnecessarily, by identifying alternative support systems and preventive measures. The position requires a client-centered approach that respects self-determination, involves them in decision-making, and empowers them to achieve their goals. Effective collaboration with community partners and service providers is essential for providing holistic support and addressing any gaps in care. The role also involves maintaining accurate documentation and preparing reports to ensure compliance with organizational policies and funding requirements

Requirements

  • A bachelor's degree in human services/ related field or commensurate experience
  • 2+ years of street outreach/case management/ direct services/other related experience
  • Strong written and oral communication skills
  • Ability to prioritize competing priorities and make sound judgements
  • Ability to complete tasks while navigating frequent interruptions
  • Ability to deliver creative, resourceful solutions to unique challenges
  • Strong organizational and time management skills
  • Valid Driver’s License and reliable transportation
  • Ability to Drive a 15-passenger van
  • Willingness to work evenings and weekends when needed
  • Annual TB Test is required
  • Ability to sit or stand for long periods
  • Ability to lift items weighing 10-20 pounds

Responsibilities

  • Participant-Centered Support Coordinate and manage assigned prevention cases
  • Develop, maintain, and share a comprehensive resource library and related communication materials.
  • Promote program awareness and build trust with community partners to enhance engagement.
  • Provide Housing location services, referrals, and advocacy
  • Facilitating housing-related tasks such as document collection
  • Assisting with rental assistance packages from various funding sources, properly documenting fund distribution, and leading the recertification process as needed.
  • Address safety concerns quickly and compassionately.
  • Collaboration & Administrative Support Collaborate as part of a multidisciplinary team to ensure timely service delivery.
  • Ensure all relevant participant information is promptly shared with the appropriate staff.
  • Monitor participant progress through regular contact and communication with external and internal service providers, conducting assessments, and making necessary referrals.
  • Manage tasks and maintain up-to-date, complete participant charts, including scanning documentation as needed.
  • Ensure accurate and timely documentation of participant interactions within the HMIS database.
  • Perform monthly data clean-up to maintain HMIS accuracy.
  • Participate in weekly staff meetings and training sessions as directed by the supervisor.
  • Proactively communicate caseload coverage and provide updates on outstanding tasks when absent from work.
