About The Position

The Preschool Services Coordinator is responsible for coordinating services and support for consumers as outlined in the Lanterman Developmental Disabilities Services Act. This position involves evaluating consumer progress, maintaining contact with consumers and their families, providing advocacy services, and coordinating assessments. The role requires travel within the San Bernardino South West area, and the position does not have supervisory responsibilities.

Requirements

  • Master's Degree in social work, psychology, public health nursing, teaching special education, early childhood development, or related field from an accredited college or university.
  • OR BA or BS degree from an accredited college or university and one year of experience in case management, intellectual/developmental disabilities, social work, special education, or early childhood development.
  • Bilingual preferred.
  • Ability to handle change well and be flexible.
  • Ability to follow oral and written direction.
  • Ability to establish and maintain effective working relationships.
  • Good verbal and written communication skills.
  • Possession of a valid California driver's license and liability insurance.

Responsibilities

  • Assume daily Consumer Services Coordinator responsibilities under the direction of the Program Manager.
  • Evaluate consumer progress and plan as identified in Individual Program Plan (IPP) on an ongoing basis.
  • Maintain contact with consumers and their families as required.
  • Provide advocacy services to consumers and their families, attending relevant meetings and preparing necessary paperwork.
  • Seek out and utilize generic resources on behalf of consumers.
  • Perform individual or family crisis intervention and document activities appropriately.
  • Complete special incident reports on a timely basis.
  • Coordinate consumer assessments including nursing, medical, psychological/developmental, education or vocational.
  • Provide case coordination coverage for absent Consumer Services Coordinators.
  • Coordinate, implement and update the CDER, Annual Review and IPP process.
  • Serve Medicaid Waiver cases completing required documentation.
  • Act as the program's/regional center's liaison to community agencies and parent groups.
  • Provide written reports of activities to the Program Manager at least annually.
  • Arrange for placement of consumers in residential facilities as needed.
  • Represent the regional center at case conferences and interagency meetings.
  • Keep Program Manager informed of the status of the caseload and community needs.
  • Act as the program sponsor for new employees, providing training in agency philosophy and procedures.
  • Lead or participate in Quality Assurance evaluations or audits.
  • Complete administrative requirements and documentation in accordance with agency policies.
  • Maintain an adequate skill level in interpersonal and community relationships.
  • Comply with the Agency's Injury and Illness Prevention Plan.
  • Utilize agency's IT systems and maintain security.
  • Organize travel efficiently and maintain a safe driving record.
