Your Role as a Home Health RN Preceptor
As a Preceptor, you will serve as a clinical leader and educator, supporting both new hires and experienced clinicians while maintaining a reduced patient caseload.

Requirements

• Active, unencumbered Florida RN license (required)
• Minimum 2 years of home health experience (required)
• Proficiency with Medicare OASIS documentation (required)
• Experience with WellSky/Kinnser EMR (required)
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to travel within assigned region daily

Responsibilities

• Conduct co-visits with clinicians to provide real-time training and support
• Lead local clinical orientation and onboarding activities
• Complete competency and skill validations for nursing staff
• Provide ongoing mentorship and clinical guidance to field clinicians
• Travel to branches across Clay, Flagler, and Volusia counties as needed
• Perform patient assessments, OASIS documentation, and care planning
• Deliver skilled nursing care including:    – Chronic and acute disease management    – Medication management    – Wound care and post-surgical support    – Pain management
• Collaborate with physicians and interdisciplinary teams
• Support quality, compliance, and clinical excellence initiatives
