Responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes. The Coordinator will be part of a multidisciplinary team dedicated to coordination of optimal care for traditional Medicare patients undergoing one of five specified surgical procedures, providing support throughout the patient’s care journey. The Coordinator will be the primary contact for the patient and their family, and they will perform extensive outreach to patients. They will answer patient questions at each step, providing crucial care navigation support for the patient and family. The Coordinator will serve as a central member of the team and provide direct service to both patients and their caregivers. The Coordinator will be responsible for scheduling regular check-in meetings with the patient, and appointments with the CMS TEAM APP as needed. They will schedule, confirm and document all pre-op appointments and goals, and all post-op enrollment and attendance as appropriate. They will work with the patient to complete intakes and assessments, and provide education and coaching support, as well as enrolling patients in education classes. They will also support coordination across all members of the multidisciplinary team.

  • Advocate for patients and their families, ensuring their needs and preferences are considered in the care planning process.
  • Serve as a liaison between patients, healthcare providers, and other relevant stakeholders.
  • Collaborate with the interdisciplinary healthcare team to coordinate patient care services, appointments, and follow-up plans.
  • Assist patients in understanding and adhering to their care plans.
  • Identify and connect patients with appropriate healthcare and community resources, such as support groups, financial assistance programs, and transportation services.
  • Educate patients and their families about their medical conditions, treatment options, and self-care strategies.
  • Ensure that patients are informed and empowered to actively participate in their healthcare decisions.
  • Maintain accurate and detailed documentation of patient interactions, care plans, and resource referrals.
  • Bachelor's Degree Clinical Social Work required or Bachelor's Degree Healthcare Management required or Bachelor's Degree Related Field of Study required
  • Social work, case management or related field preferably in a clinical setting 1-2 years preferred
  • Strong knowledge of healthcare resources, community services, and patient advocacy.
  • Excellent communication and interpersonal skills.
  • Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences.
  • Strong organizational and time management skills.
  • Familiarity with electronic health records and case management software.
