Job Description: The Pre-Admission Coordinator plays a critical role in ensuring patients receive the right care, at the right time, in the right place. In this role, you’ll review patient records prior to admission to confirm complete clinical documentation and key information needed for both care delivery and billing. You’ll manage pending admissions, whether awaiting discharge or surgery, and partner closely with sales teams and referring facilities to ensure patients are informed, supported, and successfully transitioned into care. Requirements

  • Degree from an accredited school of professional nursing
  • Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license
  • Minimum of 1 year of practical nursing experience
  • Strong communication skills, including the ability to listen, clarify, and respond effectively
  • Ability to prioritize, plan ahead, and manage multiple tasks and deadlines
  • Confidence making decisions with incomplete information and knowing when to escalate
  • Ability to quickly learn enterprise systems such as Homecare Homebase and Workday, with intermediate Microsoft Office skills
  • Strong problem-solving, conflict resolution, and negotiation skills
  • Patient-focused mindset with the ability to manage expectations and advocate for patient needs
  • Sit, stand, bend, lift and move intermittently and be able to lift at least 25lbs
  • Valid driver’s license, auto insurance, and reliable transportation

Nice To Haves

  • Home Health or Hospice experience preferred
  • Bilingual in English and Spanish preferred

Responsibilities

  • Review and validate clinical documentation prior to patient admission
  • Manage and track pending admissions for your assigned area
  • Coordinate with sales teams and referring facilities to ensure timely, informed patient conversion
  • Communicate clearly and professionally with internal and external partners
  • Support accurate billing and seamless care delivery through strong documentation review
