About The Position

The Registered Nurse (RN) at White Plains Hospital is responsible for providing and coordinating patient care in an ambulatory setting. This role involves assessing, planning, and intervening in health problems, as well as educating patients and their families on health maintenance strategies. Requirements

  • BSN required.
  • Graduate from an accredited school of nursing with current New York RN registration.
  • BLS certification required (must be through the American Heart Association).
  • ACLS required for all RNs working in the Cancer Center or Center for Advanced Medicine & Surgery (CAMS).
  • Knowledge of current nursing, medical and health trends and practices in rendering of patient care.

Responsibilities

  • Understand and adhere to the WPH Performance Standards, Policies and Behaviors.
  • Demonstrate the ability to perform clinical/technical/service/administrative tasks.
  • Systematically and continuously assess patient physical, psychosocial and spiritual health status and knowledge base.
  • Provide and/or delegate patient care according to the care goals, setting priorities based on patient need.
  • Triage all sick and walk-in patients.
  • Establish therapeutic and supportive relationships with patients and family.
  • Act as a resource person.
  • Coordinate client care including referral back to primary provider.
  • Assess needs and initiate referrals (VNS, Social Work).
  • Perform telephone triage.
  • Handle prescription requests according to procedure.
  • Communicate normal and abnormal test results in collaboration with provider including appropriate follow-up.
  • Perform therapeutic treatments and procedures.
  • Assist with procedures.
  • Administer oral/IM/IV/SQ/Inhaled medications.
  • Coordinate, delegate and/or provide all other patient care activities.
  • Coordinate in-patient admissions and ER referrals.
  • Act as a patient advocate.
  • Document according to standards of care.
  • Promote and provide for patient safety in accordance with state and federal guidelines.
  • Participate in the department's performance improvement activities.
  • Provide presentations to care team members.
  • Ensure all staff participates as a member of the patient care team, participates in daily huddles and attend regular scheduled practice team meetings.
  • Attend in-service and continuing education programs.
  • Demonstrate a professional, courteous, and respectful attitude in dealing with patients, families and significant others.
  • Inform and educate patient/family to enable participation in their care and decision-making.
  • Evaluate the effectiveness of interventions and patient responses and outcomes to care.
  • Collaborate with team members to make necessary changes to patient plan of care.
  • Communicate pertinent information and changes in patient status to appropriate members of the care team and other health care providers.
  • Perform all other related duties as assigned.
