About The Position

We are seeking a highly motivated and compassionate Advanced Practice Provider (APP) to join our multidisciplinary Post-Transplant team. The APP will play a vital role in the ongoing care and management of patients following solid organ transplantation (e.g., kidney). This provider will work collaboratively with transplant physicians, surgeons, nurses, pharmacists, and other care team members to ensure the best possible outcomes for post-transplant patients.

Requirements

  • Master’s or Doctoral degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.
  • Current and valid licensure as an NP or PA in New York.
  • National certification (e.g., ANCC, AANP, NCCPA).
  • DEA license (or eligibility to obtain).
  • BLS and ACLS certification required.
  • 1–2 years of experience in transplant medicine, hepatology, nephrology, or critical care preferred.
  • Strong clinical judgment and critical thinking skills.
  • Excellent communication and patient education abilities.
  • Ability to work independently and as part of an interdisciplinary team.
  • Proficiency in electronic medical records and telehealth platforms.
  • Sensitivity to diverse patient populations and a commitment to equitable care.

  • New graduates with strong clinical backgrounds in transplant or acute care may be considered.

Responsibilities

  • Provide comprehensive post-transplant care, including assessment, diagnosis, treatment planning, and follow-up management.
  • Monitor for transplant-related complications such as rejection, infection, or medication side effects.
  • Adjust immunosuppressive and supportive therapies under established protocols or provider supervision.
  • Order and interpret diagnostic studies, including lab work and imaging.
  • Collaborate with transplant coordinators, social workers, dietitians, and other team members to support patients' clinical and psychosocial needs.
  • Educate patients and families on post-transplant care, including medication adherence, lifestyle changes, and signs of complications.
  • Participate in transplant team meetings and quality improvement initiatives.
  • Maintain thorough and timely documentation in the electronic medical record (EMR).
  • May assist with hospital rounding, outpatient clinics, and telephone/on-call triage depending on role setting.
